Parent Forms

Parent and caregiver forms provide the school with important information needed to support student safety, well-being, learning, and participation in school activities. We encourage families to complete all required forms promptly and to review them each year to ensure information is accurate and up to date. Timely completion of forms helps our staff communicate effectively with families and provide the best possible support for every student.


Parents/Guardians are asked to complete the following forms during the first 2 weeks of the school year - September 18th, 2026

Forms coming home with students:
1. Reunification Form (Will be distributed to students - digital copy below) - Required
2. Personal Information Verification Form (Will be distributed to students - individualized) - Required
3. Medical Information Sheet (Print the file and bring to office if your child has medical condition)  ( )- Required
4. Windermere Field Studies Neighbourhood Consent (Will be distributed to students - digital copy below) - Recommended

1. CASL VSB Consent - Recommended

The VSB CASL Consent Form is a consent form related to Canada's Anti-Spam Legislation (CASL). By completing this form, parents and caregivers give permission for the 自拍流出 and their school to communicate with them electronically through email regarding school news, newsletters, events, reminders, field trips, and other important school information.


2. VSB Media Consent Form - Recommended

The VSB Media Consent Form gives parents and caregivers the opportunity to decide whether their child's image, name, voice, or work may be used by the 自拍流出 for educational, informational, or promotional purposes. This may include photographs, videos, audio recordings, student work, school websites, district publications, newsletters, and social media channels.


3. External Media Consent Form 

The VSB Media Consent allows newspapers, television stations, radio broadcasters, and other media outlets to photograph, film, or interview students when covering school activities and achievements. The purpose is to promote public understanding of school programs, celebrate student success, and support public education.
